System of Care & Community Collaboratives
Promoting Each Child's Potential
System of Care (SOC) is group of community services and supports who help families of children with behavioral health needs.
The SOC model understands that family, school, and community are all important. It helps each child reach their full potential by focusing on their strengths. SOC brings together support from many parts of a child’s life, including physical health care, mental health care, and social services.
Each county in the Trillium area has a System of Care Coordinator. These coordinators know their counties well and know what services and supports are available. They are important resources in their communities and take part in different teams and committees such as:
- Juvenile Crime Prevention Committees (Department of Public Safety)
- Child Fatality Review Teams (Public Health Department)
- Community Collaborative Meetings

Community Collaboratives
Community Collaboratives are groups made up of people from public agencies, commnity groups, faith-based organizations, and other local resources. They work together to find gaps in service and support and create solutions to help individuals and families with complex needs.
Each county in the Trillium area has its own Community Collaborative that meets on a regular basis. System of Care Coordinators support the leaders of these groups, provide guidance and help to community partners, and share information from Trillium during Community Collaborative meetings.
