Stated-Funded Recipients Information

The Trillium Tailored Plans serves the complex needs of individuals with significant behavioral health disorders, intellectual and developmental disabilities, and traumatic brain injury — populations Trillium and all other LME/MCOs have long served. The Trillium Tailored Plans also serves people who are uninsured and getting state-funded services, regardless of diagnosis.

What to know about the Trillium Tailored Plan launches?

The Trillium Tailored Planoffers a variety of services and supports for those receiving state-funded services. . Here is some information about the Trillium Tailored Plan:

  • A dedicated handbook for recipients to review appropriate information and requirements.
  • We help educate about ways to improve your well-being through health promotion materials (such as diabetes resources), health prevention, and a tobacco cession program to stop smoking.
  • Core services available including diagnostic assessments, facility-based crisis for adults, mobile crisis management, opioid treatment, outpatient services, and more.
  • Case management for adults and children with behavioral health needs, or care management for adults and children with I/DD or TBI. These staff can help make appointments, answer your questions, and connect you to resources in your communities. 

Information for Recipients

State-Funded Services are not an entitlement and funding is limited. This means that even if you are eligible to get services, if the funding runs out they may not be available. If this happens, you may be put on a waiting list by Trillium until additional funds are available. If you are pregnant and using drugs or alcohol, or are a person who injects drugs, you may be able to get help while you wait for services. Please contact your care manager or Member and Recipient Services at 1-877-685-2415 to let them know about any pregnancy and/or substance use.
