Trillium has identified a need for Psychosocial Rehabilitation service in Currituck and Dare Counties. Psychosocial Rehabilitation service is designed to help adults with mental health and other psychiatric concerns increase their functioning so that they can be successful and satisfied in the environment of their choice with the least amount of ongoing professional intervention. Psychosocial Rehabilitation focuses on recovery, skill and resource development related to life in the community and to increasing the participant’s ability to live as independently as possible, to manage their lives with as little professional intervention as possible, and to participate in community opportunities related to vocational, functional, social and educational goals.

Providers must be enrolled in NC Tracks and meet all requirements in Clinical Coverage Policy 8A

These services focus on reducing psychiatric and behavioral symptoms to improve functioning in familial, social, educational, or occupational life domains. Providers must be enrolled in NC Tracks and meet all requirements in Clinical Coverage Policy 8C.

• Psychiatric Mental Health Nurse Practitioner (PMHNP)

Trillium is preparing to operate under the TBI Waiver and will award contracts as services are added to the Trillium Benefit Plan. This recruitment aims to expand the Trillium Health Resources network of qualified, approved NC Medicaid providers experienced in providing high-quality services to Trillium Members. Priority consideration will be given to licensed Neuropsychologists due to their specialized expertise in assessing and treating cognitive impairments related to TBI. Interested providers must have national accreditation to provide waiver services and must apply for consideration for the following service needs:

Important Information:

• Provider MUST demonstrate experience serving members with Traumatic Brain Injury.
• Provider MUST meet all qualifications of the service listed in Appendix C (Page 54) for the Waiver Amendment at  Application for 1915(c) HCBS Waiver: NC.1326.R01.04 - Mar 01, 2024 (as of Mar 01, 2024) (ncdhhs.gov) and NC Medicaid to be eligible for a contract award.
• Recruitment is for Medicaid services only.
• No start-up funding is available from Trillium. 
• Provider must be fiscally sound and have capital to start programs independently.
• Application does not guarantee a contract. Providers are advised to wait until the official contract is awarded before implementing programs.
• Provider must be an approved NC Medicaid provider enrolled in NC Tracks.
• More than one provider may be selected for a service if warranted based on member needs.

Trillium is seeking providers to deliver state-funded and Medicaid Tailored Plan and Medicaid Direct Mobile Crisis services in specified counties.   The coverage area for this RFA includes Sampson, Bladen, Columbus, Robeson, Scotland, Hoke, Lee, Moore, Anson, Richmond, Montgomery, Randolph and Guilford counties

Rural Equity and Innovation Integrated Care (REI-IC) will increase capacity to meet integrated care needs of southeastern and south central rural NC and address health disparities in this region by paying for equity for Tailored plan members living in this region. This RFA will award 1M in funding for 6 providers. The model will evaluate the impact of a value-based care (VBC) approach focused on Tailored Plan members. The model will provide access to primary care treatment through an integrated care delivery framework located in a specialty BH community based organization. The organization will have a primary focus on members with moderate to severe mental health conditions and/or chronic substance use disorders and will include members with dual IDD diagnosis. Members will have access to primary care services in the same setting as behavioral health services.

The primary outcome is to increase access to integrated care in Tier I  counties in rural NC. Other objectives of this demonstration pilot also include improving quality of care, timely access to appointments for Tailored plan members, reducing avoidable emergency department and inpatient utilization, and strengthening telehealth systems capacity.

Trillium will award funding for up to six provider for this pilot.

The purpose of this Request for Proposal (RFP) is to invite service providers to submit a proposal for the implementation of Transition Management Services (TMS). There are five regions/counties included in this RFP as follows: 

1. Craven,
2. Guilford/Randolph (Closed, Contract Pending),
3. Moore/Hoke,
4. New Hanover,
5. Pitt/Greene/Wilson (Closed, Contract Pending).

Providers may submit proposals for one or more of the counties/regions included in the RFP.

Trillium Health Resources is actively seeking organizations that are interested in conducting Trauma Informed Comprehensive Clinical Assessments (TiCCAs) as part of the Standardized Assessment Process (SAP) with Wilson County Department of Social Services (DSS) and the surrounding counties.

The TiCCAs will be for youth impacted by Wilson County DSS in an effort to ensure proper diagnosis, leading to proper service provisions subsequently enhancing the outcomes for both children and their families. The assessments are developed by Benchmarks NC, who will also provide the appropriate trainings for clinicians to perform the TiCCAs. 

Trillium will choose two provider agencies in Wilson County and the surrounding counties to conduct the TiCCAs. The selected provider agencies will designate a clinical team leader to oversee the project and the clinical team. The clinical team will be comprised of no more than five additional clinicians who will also participate in the project.

Trillium has identified a need for Psychosocial Rehabilitation in the provider network for both Currituck and Dare Counties. Trillium will support the expansion of this service in either of these counties with Medicaid funding. Trillium Health Resources will work closely with the selected provider to ensure that the service is being provided as clinically indicated.
