Physical Health Services

Trillium in-network physical health providers may contact Carolina Complete Health to request changes to a Trillium Tailored Plan physical health contract. 

For physical health contract changes, please contact Carolina Complete Health directly via email networkrelations@cch-network.com or call 1-833-552-3876 (after choosing Provider Services, press 8 for Contracting).

Behavioral Health, Substance Use, and Intellectual and Developmental Disabilities Services

Trillium in-network providers requesting site and/or service additions should email NetworkServicesSupport@TrilliumNC.org for assistance except to add TFC/IAFT Sites and/or AFL/Respite Sites. For these locations, providers should use the applicable forms located on our Provider Documents Forms webpage.

*Trillium operates a closed network for all behavioral health, substance use, and intellectual and developmental disabilities services. Trillium will not approve any requests to add state-funded services due to lack of available funding.
