Empowering Access to Care through Transportation
Since July 1, 2024, over 8,000 members have safely arrived at more than 472,000 doctor appointments, thanks to Trillium’s coordinated transportation services. For many, these rides meant catching early signs of illness, staying on top of chronic conditions, or simply being able to get the care they need without barriers. Every ride is a step toward stronger mental and physical health—and toward brighter, healthier futures.

Innovation That Supports Independent Living
Through the Tailored Plan, technology is transforming independence for our members. Tools like the Trillium Ultimate Living Assistant (TULA), Attend Behavior, CHESS, and smart home communities like Francis’ Cove are empowering members to take control of their health. Whether it's setting medication reminders, communicating instantly with care teams, or receiving real-time remote support, these innovations are helping members live safely, confidently, and independently in their communities.

Whole-Person Support through Tailored Care Management
Today, nearly 100,000 members are receiving personalized support through Tailored Care Management—22,637 with Trillium’s own TCM teams and 75,474 through our partners. Behind each number is a story: a person managing diabetes more effectively, a parent finding the right pediatrician for their child, or an individual getting help navigating complex systems. Our Care Managers are true partners, guiding members toward healthier, more stable lives every day.

Opening Doors to Second Chances: Re-entry and Recovery Support
Life after incarceration comes with enormous challenges. Since the Tailored Plan began, over 1,000 community leaders have participated in our Re-entry Simulations, building awareness of the barriers individuals face. Our STAR team has walked alongside over 300 individuals returning home, connecting them to housing, benefits, treatment, and support. Thanks to this compassionate care, STAR participants have 40% lower recidivism rates—proving that when we believe in people, communities are stronger.

Changing Lives through Employment
Employment offers not just income, but purpose, dignity, and connection. Since July 2024, 331 members have worked with Trillium’s IPS-Supported Employment providers, and 181 individuals (nearly 58%) have secured or maintained competitive jobs—proving that meaningful work is possible for everyone, no matter the challenges they face. This is hope in action—and we celebrate every member’s courage and resilience.

Expanding Opportunities through Innovations Waiver
198 individuals have been newly onboarded to the Innovations Waiver through Trillium’s Care Management, opening doors to services that help them live, work, and thrive in their chosen communities. These services are about more than care—they’re about dreams, dignity, and the power of self-direction.

Building Strong Partnerships for Member Care
Behind every member’s care is a network committed to their success. In just one year, Trillium has partnered with 169 Behavioral Health and 967 Physical Health providers, ensuring that members have access to the right care, at the right time, close to home. Together, we are breaking down silos and creating truly integrated, person-centered care.

Investing in Our Workforce to Better Serve Members
Behind every great outcome are the professionals who make it possible. Trillium has trained 718 Tailored Care Management professionals, equipping them with the skills, compassion, and cultural responsiveness needed to serve members with complex needs. This ongoing investment ensures that every member is met by a care team that sees the whole person and walks alongside them on their journey to wellness.

Trillium's Specialty Care Team for Innovations Waiver
At Trillium, we believe in empowering every individual to live a life of greater independence and community connection. Our dedicated Specialty Care Team for Innovations Waiver goes beyond traditional services, providing vital support for needs that truly transform lives. From ensuring safe homes and accessible transportation through crucial home and vehicle modifications (our most frequent requests, making up 395 of 4,242 impactful solutions!) to delivering essential equipment, nutritional supplies, and critical connectivity, we're building bridges to a richer, more engaged life. We also foster growth through Community Networking classes and vital education on natural supports, enriching experiences and helping members and their families thrive within their communities.

Community Outreach and Engagement
Our Regional Operations team is the heartbeat of Trillium’s commitment to our communities, fostering profound change through unwavering engagement. Every month, we actively participate in an average of 422 meetings, a testament to our robust, ongoing collaboration with cherished local county and community partners, dedicated providers, and vital local stakeholders. We also lead approximately 36 impactful presentations monthly, sharing crucial updates, shaping progressive policies, and delivering empowering training that ensures the seamless implementation of the Tailored Plan. These relentless efforts highlight our unwavering dedication to transparent communication, preparing every stakeholder for success, and championing truly person-centered care that puts individuals at the heart of everything we do.

Paid Claims
While numbers might seem impersonal, our paid claims data paints a powerful picture of profound impact and expansive reach within the Trillium Tailored Plan. From July 1, 2024, through May 31, 2025, we facilitated an astounding 5,861,496 claims, totaling over $1.6 billion in vital support. This includes 2,411,396 claims dedicated to transformative behavioral health services, 2,086,324 for essential physical health care, and 1,363,776 for life-changing prescriptions. Millions of visits, backed by over a billion dollars, are not just transactions; they are investments in brighter futures, contributing directly to better health outcomes, more inclusive living environments, proactive preventative care, and deeply enhanced community engagement for countless individuals. This data isn't just about volume; it's about the lives we're touching and the healthier, more vibrant communities we're building together.
